Documents required for candidates for 2025/2026 studies

From the candidates for our first cycle studies we require:
personal questionnaire, including survey (printed from USOS website)
copy of the secondary school leaving certificate (the original version should also be included for inspection only) which entitles the graduate to study in Poland
document confirming the knowledge of Polish language
one photography 35x45 mm, taken with the accurate requirements for the Polish ID
certificate from the occupational medicine doctor, which indicates that there are no medical contraindications for taking up the studies (referrals available at the University)
From the candidates for our second cycle studies we require:
personal questionnaire, including survey (printed from USOS website)
copy of bachelor degree certificate (the original version should also be included for inspection only)
copy of the Diploma’s Supplement- applies only to students who studied in the years when such were issued, in case of the rest of students who did not get it - extract from index.
document confirming the knowledge of Polish language
one photography 35x45 mm, taken with the accurate requirements for the Polish ID
certificate from the occupational medicine doctor, which indicates that there are no medical contraindications for taking up the studies (referrals available at the University)
Important information:
for each document, which was created in foreign language certified translation in Polish has to be attached
certificates of the studies from abroad or certificates different than Matura Exam should have apostille or go through legalization process
