From the candidates for our second cycle studies we require:
- personal questionnaire, including survey (printed from USOS website)
- copy of bachelor degree certificate (the original version should also be included for inspection only)
- copy of the Diploma’s Supplement- applies only to students who studied in the years when such were issued, in case of the rest of students who did not get it - extract from index.
- document confirming the knowledge of Polish language
- one photography 35x45 mm, taken with the accurate requirements for the Polish ID
- certificate from the occupational medicine doctor, which indicates that there are no medical contraindications for taking up the studies (referrals available at the University)
Important information:
- for each document, which was created in foreign language certified translation in Polish has to be attached
- certificates of the studies from abroad or certificates different than Matura Exam should have apostille or go through legalization process
